Mark Adams, Pharm.D.
Mark Adams is a clinical pharmacist at Elliot Hospital and currently works on the Medical and Surgical floors, Cardiac Intermediate Care Unit, Intensive Care Unit, Emergency Department, and the Central Pharmacy. Dr. Adams serves as an infectious disease preceptor for PGY-1 residents.
After graduating with his Doctor of Pharmacy from University of New England in 2021, he completed his PGY-1 pharmacy residency at Elliot Hospital in 2022. His primary interests are infectious diseases and critical care. He is a member of the Infection Prevention and Antimicrobial Stewardship Committee.
Michelle Andrade, Pharm.D.
Michelle Andrade is the pharmacy supply and systems analyst at Elliot Hospital. She supports the clinical team by managing formulary items within clinical informatic systems for appropriate implementation, operation, and industry compliance. Dr. Andrade serves as the pharmacy informatics preceptor for PGY-1 residents.
Dr. Andrade earned her Doctor of Pharmacy and Bachelor of Science in chemistry from Massachusetts College of Pharmacy and Health Sciences in Boston. She completed her PGY-1 and pediatric PGY-2 residencies at Elliot Hospital. She received the AACP Education Scholar Teaching Certification through the University of New England. She subsequently earned her board certification in pediatric pharmacy during her tenure as a pediatric clinical pharmacist at Elliot, before moving into program and system management roles. Her current areas of interest include informatic efficiency, applied machine intelligence, and root cause analysis. She serves as an IT liaison in support of system troubleshooting, product build, and shortage management.
Melissa Crockett, Pharm.D., BCPS
Melissa Crockett is the pharmacist supervisor at Elliot Hospital and currently works on the Medical and Surgical floors, the Cardiac Intermediate Care Unit, the Emergency Department, the Intensive Care Unit, and the Central Pharmacy. She joined the Elliot Hospital team in 2017. Dr. Crockett serves as an infectious disease and antimicrobial stewardship preceptor for PGY-1 residents. She is also an internal medicine preceptor for APPE students.
As a native of Maine/New Hampshire, Dr. Crockett earned her Doctor of Pharmacy from the Massachusetts College of Pharmacy and Health Sciences-Manchester campus and holds a bachelor’s in biology with a minor in medical laboratory science from the University of New Hampshire. Her current areas of interest include internal medicine and infectious disease. She is a member of the American Society of Health-System Pharmacists.
Katie Fitzgerald, RPh, BCPS
Katie Fitzgerald is a clinical pharmacist at Elliot Hospital. Katie currently works in the Pediatric Unit, Neonatal ICU, Cardiac ICU, Medical Surgical floors, and the Emergency Department. She is actively involved in the Medication Safety Committee, the Code Blue Committee, and is a member of the American Society of Health-System Pharmacists. She precepts residents during the orientation and staffing rotations of the residency.
Katie earned her B.S. from Northeastern University. She has 21 years of experience as a hospital pharmacist, is a Board-Certified Pharmacotherapy Specialist (BCPS), and has precepted students from Massachusetts College of Pharmacy, University of Connecticut, and University of Rhode Island. Her current areas of interest include pediatrics and medication safety.
Donna Grant, Pharm.D.
Donna Grant is a clinical pharmacist at Elliot Hospital. Donna currently staffs in the Pediatric Unit, Neonatal ICU, Cardiac ICU, Medical Surgical floors, and the Emergency Department. She is actively involved in the IRB, student precepting, USP 797 & IV Room Committee, and the USP 800 Compliance Team. She is a member of the New Hampshire Pharmacist Association. She precepts residents during the orientation and staffing rotations of the residency.
Donna earned her Pharm.D. from Massachusetts College of Pharmacy in Boston. She has 16 years of experience as a hospital pharmacist and 20 years of experience at Elliot Hospital. She precepts IPPE pharmacy students from Husson University, Massachusetts College of Pharmacy in Manchester, University of Connecticut, and the University of New England. Her current areas of interest include IV room operations and USP 797 and 800 and oncology.
Jacob Hoffmeier, Pharm.D.
Jacob Hoffmeier is a Clinical Pharmacist at Elliot Hospital and currently staffs on the Intensive Care Unit, Medical and Surgical floors, the Cardiac Intermediate Care Unit, NICU/Pedi, Cancer Center, the Emergency Department, and the Central Pharmacy. He joined the Elliot Hospital team in 2020. Dr. Hoffmeier serves as a nutrition and surgical services preceptor for PGY-1 residents. He is also an internal medicine preceptor for APPE students.
Dr. Hoffmeier earned his Doctor of Pharmacy from the University of Kansas School of Pharmacy and holds a B.S. in microbiology from Kansas State University. He completed his PGY-1 pharmacy residency postgraduate training at Elliot Hospital in 2020. His current areas of interest include intensive care, cardiology, and infectious disease.
Joseph Kvetkosky, Pharm.D.
Joseph Kvetkosky is a Clinical Pharmacist at Elliot Hospital in Manchester, NH. Dr. Kvetkosky earned his Doctor of Pharmacy degree from the University of Rhode Island. He has over 20 years of experience as a pharmacist along with multiple years of precepting pharmacy students and residents.
Dr. Kvetkosky currently staffs in the Neonatal ICU, Cardiac ICU, Emergency Department, Post Surgical and Medical Surgical floors. He serves as a Nutrition preceptor for PGY-1 residents. His current areas of interest include neonatal care, clinical nutrition and infectious disease.
Christopher Lopez, Pharm.D., BCACP, CDCES
Christopher is a two-time graduate of the University of Connecticut, where he most recently received his Doctor of Pharmacy (PharmD) degree. He has worked in many areas of Pharmacy practice including community, academia, inpatient, and ambulatory. He is a Certified Diabetes Care and Education Specialist (CDCES) and a Board-Certified Ambulatory Care Pharmacist (BCACP). Currently, Chris works as the Ambulatory Pharmacy Clinical Coordinator at the Elliot Health System where he has been charged with building a comprehensive ambulatory practice across the system. Chris also has per diem positions as a Pharmacist Consultant for the NH Division of Public Health Services and as the appointed Health Officer for the town of Warner, NH. Chris is currently a graduate student at the University of New Hampshire’s School of Health Management & Policy where he anticipates earning a Master of Public Health (MPH) in the near future.
Christopher has previously served on one of the American Society of Health-Systems Pharmacists’ (ASHP) Ambulatory Care Section Advisory Groups, as well as the Certification Board for Diabetes Care and Education’s (CBCDE) Examination Committee and is a past president of the New Hampshire Pharmacists Association (NHPA). His clinical interests include management of chronic disease, public and population health, process improvement, and practice design.
He lives in Warner, NH with his wife Alicia, who is also a pharmacist, and their three children, Caleb, Tanner, and Lena.
Carly Magoon, Pharm.D., MPH
Carly Magoon is a Clinical Pharmacist at Elliot Hospital where she practices in the Intensive Care Unit, Cardiac Intermediate Care Unit, Emergency Department, and general medicine floors. Carly received her Doctor of Pharmacy and Master of Public Health degrees from MCPHS University in Boston. She completed her PGY-1 Residency at Elliot Hospital in 2022.
Clinically, Carly is interested in critical care, pain management, and opioid stewardship. Carly precepts longitudinal APPE pharmacy students on their critical care elective and is an adult Critical Care I preceptor for PGY-1 residents. She is also chair of the Opioid Stewardship Committee.
Ian Maltz, Pharm.D.
Ian Maltz is a clinical pharmacist at Elliot Hospital and currently staffs in the Intensive Care Unit, the Emergency Department, Medical and Surgical floors, the Cardiac Intermediate Care Unit, and the Central Pharmacy. He joined the Elliot Hospital pharmacy team in 2022. Dr. Maltz serves as an internal medicine preceptor for PGY-1 residents.
Dr. Maltz earned his Doctor of Pharmacy from the University of Florida College of Pharmacy in 2021 and holds a bachelor’s in biology from Florida State University. He completed a PGY-1 residency program at Winter Haven Hospital in 2022. His current areas of interest include emergency medicine, infectious disease, and intensive care.
Kevin McKeon, RPh, BCOP
Kevin McKeon is a clinical oncology pharmacist at The Elliot Hospital Solinsky Cancer Center. He earned his B.S. at Massachusetts College of Pharmacy in 1985, and has many years of experience in the acute care hospital pharmacy setting. He has specialized in oncology for the last 15 years. He is a board-certified oncology pharmacist and a member of the American Society of Health-System Pharmacists (ASHP) and Hematology/Oncology Pharmacy Association (HOPA). He joined the Elliot Hospital staff in 2020, and precepts PGY-1 residents in the resident oncology rotation. Before Elliot Hospital, he worked many years at Beth Israel Deaconess Medical Center in Boston as clinical lead of the Oncology Management System. His current interest is optimizing oncology treatment plans for cancer center patients.
Maria Moniz, Pharm.D.
Maria Moniz is a clinical oncology pharmacist at Elliot Hospital and currently staffs at the Elliot Hospital Solinsky Cancer Center, on the Medical and Surgical floors, the Cardiac Intermediate Care Unit, the Emergency Department, and the Central Pharmacy. She joined the Elliot Hospital team in 2021. Dr. Moniz serves as an oncology preceptor for PGY-1 residents. She earned her Doctor of Pharmacy from the Massachusetts College of Pharmacy in 2008. Her current areas of interest include pharmacogenomics and expanding oncology clinical services at Elliot Hospital.
Jonathan Napoli, Pharm.D., MHA, BCPS
Jonathan Napoli is the pharmacy manager – acute care at the Elliot Hospital. He staffs on the Medical and Surgical floors as well as in Central Pharmacy operations. Dr. Napoli began his pharmacy career at Elliot Hospital in 2010. Prior to returning to Elliot in his current capacity, he worked as clinical pharmacy manager at Parkland Medical Center and director of pharmacy at Frisbie Memorial Hospital. Dr. Napoli serves as the preceptor for the longitudinal practice management rotation.
Dr. Napoli earned his Doctor of Pharmacy from Northeastern University and his MHA from Walden University. He is a member of the New Hampshire Society of Health-System Pharmacists (NHSHP) and the American Society of Health-System Pharmacists (ASHP).
John Pernyeszi, M.S., Pharm.D., BCPS
John Pernyeszi is a clinical pharmacist at Elliot Hospital and currently staffs on the Medical and Surgical floors, the Cardiac ICU, the Emergency Department, and the Central Pharmacy. He joined the Elliot Hospital team in 2018. Dr. Pernyeszi serves as an internal medicine preceptor for PGY-1 residents. He is also an internal medicine preceptor for APPE students. Dr. Pernyeszi is a Board-Certified Pharmacotherapy Specialist (BCPS).
Originally from Connecticut, Dr. Pernyeszi earned his Doctor of Pharmacy from the University of Saint Joseph School of Pharmacy and his bachelor’s and master’s in biology from Fairfield University and Southern Connecticut State University, respectively. He completed his PGY-1 pharmacy practice residency at Waterbury Hospital in Waterbury, Connecticut. His current areas of interest include internal medicine, pain management, and informatics. He is a member of the American Society of Health-System Pharmacists.
Breanne Piazik, Pharm.D., MPH, BCPS
Breanne Piazik is the medication safety officer for the Elliot Health System. Her areas of involvement include the Medication Safety Committee, Pharmacy and Therapeutics Committee, Anticoagulation Work Group, and Elliot Drug Diversion Task Force. Dr. Piazik serves as the medication safety preceptor for the PGY-1 residents.
Dr. Piazik earned her Doctor of Pharmacy from the University of Rhode Island and her MPH from Dartmouth College. She is a member of the American College of Clinical Pharmacy and the American Public Health Association. Her interests include adverse drug event prevention, error reduction strategies, and drug diversion prevention.
Brooke Raymond, Pharm.D., BCPPS, BCPS
Brooke Raymond is a Pediatric Clinical Pharmacist at Elliot Hospital who joined the team in 2022. She attained her Bachelor and Master of Science degrees in Pharmacology and Toxicology from the University of the Sciences in Philadelphia. She then went on to earn her Doctor of Pharmacy degree from the Massachusetts College of Pharmacy in 2011 and completed a PGY-1 pediatric pharmacy practice residency at Children’s Hospitals and Clinics of Minnesota.
Dr. Raymond is Board certified in Pediatrics and Pharmacotherapy and is a member of the American Society of Health-System Pharmacists and the Pediatric Pharmacy Association. She is actively involved in NICU medication safety and process improvement and the Pediatric Care Collaborative. She also serves as the Pediatric and Neonatal ICU preceptor for PGY-1 residents and APPE students. Her primary focus is general pediatrics and neonatal ICU, but also assists with the adult medical and surgical floors, cardiac ICU, emergency department and central pharmacy. Her current areas of interest include neonatal and pediatric medicine, emergency medicine, toxicology and informatics.
Laura Truhlar, Pharm.D., BCCCP
Laura Truhlar is the Pharmacy Clinical Coordinator at Elliot Hospital. She also practices as a clinical pharmacist with a focus in critical care. She staffs the intensive care unit, cardiac ICU, emergency department, medical and surgical floors, surgical services, and central pharmacy. Dr. Truhlar serves as the PGY-1 Residency Program Director and an adult Critical Care I and II preceptor for PGY-1 residents. She also developed and precepts the critical care elective for APPE longitudinal pharmacy students.
Dr. Truhlar earned her Doctor of Pharmacy degree from the Massachusetts College of Pharmacy and her Bachelor of Science degree in Chemistry from Tufts University. She completed a PGY-1 pharmacy practice residency at Elliot Hospital and received the AACP Education Scholar Teaching Certification through the University of New England. Dr. Truhlar is a Board Certified Critical Care Pharmacist and is also ACLS certified. Her current areas of interest include critical care, emergency medicine, well-being, and resilience. She is the leader of the Critical Care Pharmacy Team at Elliot Hospital and co-chair of the Pharmacy and Therapeutics Committee. She is also a member of the American Society of Health-System Pharmacists and American College of Clinical Pharmacy.
