The Elliot Medication Clinical Services provides medication therapy management, disease state education, and medication adherence support to patients of the Elliot Physician Network. A clinical pharmacist meets with patients one-on-one to review medications in order to provide education on medication usage, appropriate administration, and disease state management. The review also includes identification of drug interactions, adverse effects, and barriers to taking medications appropriately. Recommendations for changes in medication therapy are made to the primary care provider using the electronic medication record and are reviewed with the patient.
By adding a pharmacist to your healthcare team, you strengthen your ability to reach your treatment goals and reduce the risk of experiencing medication-related problems.
Our pharmacist will look for things like: drug interactions, side effects, unnecessary medications, potential cost savings, understanding of medication, and how you take your medications. If you have high blood pressure, high cholesterol or osteoporosis, the pharmacist will work with you to help you meet your therapeutic goals and provide you with education, monitoring and follow-up on any medication changes that are made by your primary care provider.

The Elliot Anticoagulation Clinics ("Coumadin Clinics") manage anticoagulation for patients within the Elliot Physician Network. The clinics are staffed by anticoagulation certified nurses. Anticoagulation monitoring is done by point-of-care finger-stick testing rather than a blood draw, and results are available within minutes. Medication dosages are adjusted at the time of the visit based on results and these results are automatically uploaded in the electronic medical record where the physician can view them at any time. The anticoagulation nurses also provide their patients with ongoing education related to anticoagulation monitoring, drug interactions, dietary interactions, and adverse effects.
At Elliot, we offer an innovative program designed to help you maximize the benefits of anticoagulant medication while minimizing your risk of complications.
The program is referral-based and works collaboratively with your physician to monitor your prothrombin time (PT/INR) so that you will achieve and maintain desired therapeutic levels. Our specially trained anticoagulation nurse will work one-on-one with you to monitor and counsel you for the duration of your anticoagulation therapy.
Unlike a blood draw, the test is performed using a finger stick test. The nurse will review your results with you and may adjust your dose appropriately at your appointment. Your results are automatically uploaded to your electronic medical record where your physician can review them at any time.
