What to Expect

After we receive the necessary information from your referring physician, we will phone you to schedule your first appointment and conduct a pre-assessment, all of which takes about 20 minutes.

You will be asked to arrive 15 minutes early for your first appointment to complete a brief questionnaire, use the provided link to open and print the questionnaire. These 7 questions pertain specifically to your pain. If you complete the questionnaire and are bringing it with you to your first appointment you need not arrive 15 minutes early.

We are committed to learning as much as we can about our patients so that we can begin to formulate a treatment plan tailored to your individual needs. Because we offer comprehensive pain management, it is important that you understand that services with other health professionals may be part of your treatment plan. Typically, you will be evaluated by a pain physician and/or a nurse practitioner. Based on our assessment, your treatment plan may include interventional therapy as well as referrals to physical therapy and/or behavioral medicine.

Our goal is to work with you to develop a treatment plan that will help reduce pain, improve function, and thereby enhance the quality of your life.
