Getting the most out of your appointment

There are a few steps we’d ask you to take in order to get the most out of your appointment. First, please plan your travel to arrive 15 minutes prior to your scheduled appointment time.

It is important for us to know all your medications, so please bring a current list of all of medications (name of drug and dose) you are currently taking. Please include any over-the-counter medications or supplements.

If you have diabetes, please bring a copy of your blood sugar log and your glucose meter. If you need a
blood sugar log, click on these links for a basic blood sugar log or an insulin pump user’s log. If you are not monitoring your blood sugars regularly or keeping a log, please do so for at least a week prior to your appointment. We will be examining your feet at every visit, so please wear footwear that is easy for
you to remove.
